What is a Primary Care Network (PCN)

Park View Surgery is part of the North Doncaster Primary Care Network.

PCN contracts were offered to practices with between 30,000 – 50,000 patients, or for groups of practices covering this number of patients. The North PCN covers a population of approximately 70,000 patients. 

The aim of the PCN is to create a workforce within primary care not purely based on the traditional view as GPs and nurses. Each PCN create a team which best suites the need of their population. Our teams now include:

• Clinical Pharmacists
• Pharmacy technicians
•  Care Co-ordinators
• First Contact Physiotherapist
• Dietician
• Mental Health Clinician

and will continue to grow as new roles are made available.   We work together under the leadership of our Clinical Directors to provide the best and most appropriate care that we can for our collective patient list.
